Position Summary:
The Home Care Worker provides person‑centered support to older people living in their own homes, assisting them to maintain independence, dignity, and quality of life. The role involves delivering practical, emotional, and personal care in line with individual care plans, organisational policies, and aged care standards.
Key Responsibilities:
Personal Care:
- Assist clients with activities of daily living, including showering, grooming, toileting, dressing, and mobility.
- Support safe transfers and use of mobility aids as required.
- Promote comfort, dignity, and respect at all times.
Domestic Support:
- Assist with light household duties such as:
- Cleaning and tidying
- Laundry and ironing
- Meal preparation in accordance with dietary requirements
- Support clients to maintain a safe and hygienic home environment
Social Support & Companionship:
- Provide companionship and emotional support
- Encourage social engagement and participation in community activities
- Accompany clients to appointments or outings when required
Health & Wellbeing Support:
- Observe and report changes in client health, behaviour, or wellbeing
- Prompt or assist with medication according to care plans (where authorised)
- Follow infection control and work health and safety procedures
Documentation & Communication:
- Complete care notes accurately and in a timely manner
- Communicate effectively with supervisors, care coordinators, clients, and families
- Adhere to confidentiality and privacy requirements
Skills and Attributes

Qualifications & Requirements:
- Certificate III in Individual Support (Ageing) or willingness to obtain
- Current First Aid and CPR certificate (or willingness to obtain)
- National Police Check
- Working with Children Check (where required)
- Current driver’s licence and reliable vehicle (preferred)
- Evidence of the right to work in Australia
Desirable Experience:
- Previous experience in aged care, home care, or community services
- Experience supporting clients with mobility issues, dementia, or complex needs
- Knowledge of Home Care Packages and person‑centred care principles
Other Requirements:
- Ability to follow care plans and organisational policies
- Commitment to professional conduct and continuous improvement
- Willingness to undertake training and professional development
